What Is A "State-Run Health Insurance Exchange?

I just realized what a health-care 'exchange' is. It hit me when seeing this piece:
Starting in October, Americans without access to affordable health care coverage through their jobs will be able to shop for insurance for themselves and their families through state-run exchanges. 
Aha! I get it now! I finally get it!

You see, an 'exchange' is an online health-care welfare application form. It's all so clear to me now. 

Good luck signing up for your health-care welfare, everyone.

Except in the majority of states where they rejected the feds supporting the exchanges for one year of 90% support because they don't accept that support would continue?
