Limbaugh picked up on this all the way back when GW picked Cheney to be his VP. He put together a montage of media coverage of the announcement showing that they all used the same template and vocabulary. He has presented other such examples from time to time ever since.

I remember hearing Limbaugh's coverage of that phenomenon.
However, in this case, it does seem this is the "Conservative" Sinclair Broadcasting Group who is responsible for this bullshit.
