He died, because he had a cardiac event. For the record, a defibrillator is only successful with certain electrical activities of the heart. It does not work against PEA (pulseless electrical activity), bradycardia (slow heart rate), junctional... electrical activity which only notes agonal responses of the dying cardiac muscle.
The classical use for a defibrillator is for V-fib, ventricular fibrillation. Was the arrest secondary to a blood clot? A pulmonary embolism?
